Revision of likely cause of steel section death at Australian engineering coy
Worksafe in Western Australia has confounded initial speculation over the cause of Wednesday's workplace fatality at Park Engineers in Welshpool, near Perth, involving an unstable steel section.
Police investigation of the incident suggested the steel had fallen some metres after a winch chain failed, Worksafe contends that the unseen accident, which left a 29-year old worker with fatal head injuries, is more likely to have been the result of the section becoming unstable and moving off a trestle while the victim was working on it.
